Antiarrhythmic drugs used in adults were introduced in children´s arrhythmia treatment, but frequently there are no available data from pediatric clinical trials. The authors present one case of amiodarone-associated arrhythmia in a neonate and the association between amiodarone and desethylamiodarone plasma concentration and the adverse event. The new-born’s follow up was made in Pediatric Cardiology Clinic from Târgu-Mureş before and after amiodarone treatment. Echocardiography, ECG, amiodarone and desetilamiodarone plasma concentrations were the followed parameters. The paroxismal supraventricular tachycardia was controlled with amiodarone, but after the increase of amiodarone dose the neonate developed a large QRS antidromic supraventricular tachycardia and the take off of amiodarone was necessary. The amiodarone and desetilamiodarone plasma concentration were very high. However, amiodarone is an effective antiarrhythmic drug in children, but the treatment can be followed by serious adverse events.
